What are Industrial Services?
Industrial Services is a specialized program that aims to help injured workers safely return to their jobs after a work-related injury, surgery, or illness. These unique subspecialties treat the whole worker by addressing physical work demands with a focus on safe, efficient return to pre-injury duty.

Bring with you your physician’s prescription, driver’s license, and insurance card. If your insurance requires a primary physician referral, it is your responsibility to obtain this prior to your appointment.
Co-pays are collected at the beginning of each visit.
Appointments
We do our best to get our patients on the schedule so they can get back to feeling their best. We understand that schedules change, and emergencies happen. We kindly ask that if you need to cancel or re-schedule an appointment that you give us at least 24 hours advance notice.
Any cancellations that occur in less than 24 hours or are missed completely will incur a $35 charge.

If you do not see your insurance company listed to the right, contact your insurance company or check their website. If we do not participate with your insurance, you can still be treated by our therapists through self-pay rates.
