Call 703-490-1112 during office hours. If the office is closed or is reached after hours, you may leave a message for the next business day. In case of an emergency, the doctor on call will be contacted.
For your visit, please bring any X-rays, MRI, CAT scans or other special tests that you have had done. The physicians require the reports as well as the films to accurately diagnose a problem.
If you have had a previous surgical procedure, please obtain any operative reports and office notes from the treating physician's office. Access to this information by our office is crucial to a complete evaluation.
If this is your first visit, we require that you bring a picture ID and your insurance card. Plan to arrive twenty minutes early for your appointment in order to complete new patient forms and prepare for X-rays if required. You may download the form now to complete prior to your appointment by clicking the blue “Patient Forms” button at the top right of the screen.
Co-pays and deductibles are required at the time of your visit. If you insurance plan is an HMO a referral is required to be seen, this should be faxed in advance or brought with you to the appointment. Please confirm that we do have this prior to your scheduled appointment time.
We are aware that emergencies arise and plans change, however, we ask for a 24 hours advance notice for cancellation. Unfortunately, if an appointment is missed or cancelled with less than 24 hours notification, we charge a $50 cancellation fee.
Nova Orthopedic and Spine Care requires that all self pay patients pay a $50 deposit to reserve an appointment slot. This fee is nonrefundable if the appointment is cancelled LESS THEN 48 hours prior to the scheduled appointment time or if the patient fails to show up for their scheduled appointment. The $50 will be credited towards any fee at the time of service.
Sometimes emergency care is required by one of our physicians which takes the physician out of the office, if this should occur we may need to reschedule your appointment and will provide you with as much advance notice as possible.
Village Physical Therapy requires that all self pay patients and high deductible insureds pay a $75 deposit to reserve an Evaluation slot. This fee is nonrefundable if the appointment is cancelled LESS THEN 48 hours prior to the scheduled appointment time or if the patient fails to show up for their scheduled appointment. The $75 will be credited towards any fee at the time of service.
FORMS AND REPORTS
We ask that you allow one week for all medical record requests or completion of any forms. All patient requested forms, as well as FMLA and disability forms require a $30 fee be paid prior to the requested forms being released. The medical records fees are set according to the Virginia statute, $10 search and handling fee and $.50 a page. All applicable fees are do prior to the release of records.
Our Surgical Coordinator is happy to assist you with any questions or concerns you have before your surgery.
All post operative instructions will be provided to you upon discharge from the facility where your procedure is performed. Once home, should you have any questions or concerns, please contact our office.
Please call during office hours to request a refill. Call prior to running out of medicine as refill requests may take 48 hours to process. Prescriptions will not be refilled after hours or on weekends.
Please notify your employer of your accident or injury as soon as possible. A claim number is required to bill your work insurance carriers. We must have the claim number and carrier information in order to pre-authorize your visit and treatment prior to an appointment being scheduled. Failure to do so may result in services not being covered by the Workers Compensation Carrier.
