New Patients:
Please call our office at (703) 490-1330 to set up an evaluation. Plan to arrive 20 minutes early to complete paperwork and review insurance coverage. Evaluations average one hour in length.
Bring with you your physician’s prescription, driver’s license and insurance card. If your insurance requires a primary physician referral, it is your responsibility to obtain this prior to your appointment.

Co-pays are collected at the beginning of each visit.
Attire:
Please wear gym clothes to your appointment, ie. sweatpants that can be pulled above the knee, shorts, t-shirts and sneakers.

We try to keep wait times to a minimum, however, emergencies occasionally arise that disrupt our schedule. Our office will try to contact you if a significant delay in our schedule occurs in order to allow for rescheduling of appointments.
If you need to change or cancel a scheduled appointment please call our office 24 hours in advance to allow us to accommodate other patients. There is a $30.00 fee for no show appointments and cancellations without 24 hour notice. Please call ahead if you will be late for an appointment as it may be necessary to reschedule this visit.
Insurance Information:
If you do not see your insurance company listed below, contact your insurance company or check their website. If we do not participate with your insurance but you have out of network benefits, you can still be treated by our therapists. Contact your insurance company and our billing department to determine your coverage.
