Office Policies

Please allow for a one week runaround period for all forms, paperwork and medical record requests. Forms may require a fee and may require payment prior too form completion. Emergency care may be required by one of our physicians and therefore some appointments may need to be rescheduled. If this occurs, our office will give as much advance notice as possible.

Making an Appointment

Office hours are Monday - Friday 8:30am - 4:30 pm.

Call 703-490-1112 during office hours. If the office is closed or is reached after hours, you may leave a message for the next business day. In case of an emergency, the doctor on call will be contacted.

If you have had X-rays, MRI, CAT scans or other special tests done, please bring all films and reports to your scheduled visit.

If you have had a previous surgical procedure, please obtain any operative reports and office notes from the treating physician's office.

Co-pays are required at the time of your visits.

If you are seeing one of our doctors for the first time, please arrive fifteen minutes early for your appointment in order to complete a new patient form. Download the form now to get started.

If you are unable to make your appointment, please notify us 24 hours in advance. Otherwise, you may be charged a fee for the missed appointment.

Pre-Post Operation Information

 

Prescription Refills

Please call during office hours to request a refill. Call prior to running out of medicine as refill requests may take 48 hours to process. Prescriptions will not be refilled after hours or on weekends.

Worker's Compensation

Please notify your employer of your accident or injury as soon as possible. A claim number is required to bill your work insurance carriers. If a claim number has not been assigned, you or your private insurance will be responsible for office charges. Our billing office will be happy to assist you in any way possible.

Insurance Participation

Insurance cards are required at the time of your visit and please bring a referral from your primary care physician if required by your health plan.
